Client Notice of Rights & Responsibilities
As a client of the Community Rightful Center, Inc., you have the following rights regarding the confidentiality of your personal information and communications with the Community Rightful Center’s staff and volunteers:
- The information that you provide to the Community Rightful Center will be kept confidential to the greatest extent allowed by law.
- The information that you provide to the Community Rightful Center, including your name, address, phone number, and other personal information will not be shared with other individuals or agencies without your permission in writing.
- The Community Rightful Center’s staff may be required by law to report certain situations even if you don’t give them permission to share or report the situations, such as suspected child abuse or neglect, threats to self or others, elder or at-risk adult abuse, commission of a crime or a civil claim against the Community Rightful Center or staff. Staff and advocates will inform you of any reporting requirements prior to having conversations with you and will tell you, when they must make a report and what information will be shared. Even when these reports are made, the Community Rightful Center, should not share information beyond what is required by law.
- Some general information about the types of services provided and overall demographics of people that use the Community Rightful Center’s services may be shared with the agencies that fund the Community Rightful Center. However, information that specifically could identify you as someone who used the Community Rightful Center’s, services will never be shared unless specifically authorized in writing by you.
- After your intake with the Community Rightful Center, you may choose to be referred to other agencies for additional help and support.
- You have the right to obtain current information concerning your personal treatment by providing a request in writing.
- You have the right to appeal any decision and/or denial of services in writing, within 30 days of the initial denial/decision.
- You have the right to report any grievances regarding services, treatment, or denial by service providers in writing.
