Case Management to assist individuals in difficult situations.

The Case Management programs at Community Rightful Center assist individuals to obtain services that would achieve a better quality of life.

Targeted Case Management aims to assist individuals in difficult situations to achieve a better quality of life. Mental Health Targeted Case Management is provided to adults with serious mental illness and children with serious emotional disturbance to assist eligible individuals in gaining access to medical, social, educational, and other services.

Minority AIDS Initiative (MAI) is an intensive medical case management to recently diagnosed HIV consumers and to those that know their positive HIV status but have not accessed medical care services for at least 6 months.

GPD Case Management promotes the development and provision of supportive housing and/or supportive services to help homeless veterans achieve residential stability, increase their skill levels and/or income, and obtain greater self-determination.

We have partnered with the Department of Veterans Affairs Health Care for Homeless Veterans (HCHV). This partnership sponsors the VA’s Homeless Providers Grant and Per Diem Program which provide services to homeless Veterans. https://www.va.gov/homeless/gpd.asp
